Our referral form makes it easy for participants, families, carers, Support Coordinators and other professionals to share the information we need to begin a conversation.
Complete the form with the participant’s consent and include as much detail as possible about their current needs, goals, preferred supports and location. Once we receive the referral, our team will review the information and get in touch to discuss suitability, availability and the next steps.

Good support starts with understanding the person behind the referral. We take time to learn about the participant’s routines, communication preferences, goals, existing supports and any concerns that should be considered before services begin.
Our team approaches NDIS referrals with care and transparency. We will confirm whether our experience, availability and support model are suitable, then explain what the onboarding process may involve.
Where appropriate, we can also communicate with family members, Support Coordinators and other providers to help create a smoother transition into support.
After receiving the referral, we will review the information provided and contact the referrer or participant to arrange an initial discussion.
This conversation gives us an opportunity to clarify support needs, confirm the participant’s location and funding arrangements, discuss preferences and answer any questions. We may then arrange a meeting or introduction before regular support begins.
We do not rush participants into a decision. Our priority is to determine whether Quality Assist Care is the right fit and make sure everyone understands what to expect before moving forward.
